Care plans/ Action Plans are for students with Allergies, Asthma, Diabetes, and Seizure disorders. Please complete the appropriate form (including a physician signature where required) and return it with any medication to the school nurse by the first day of school. 

*Note that care plans/emergency plans and medication forms must be updated annually and will not carry over from year to year.

The district encourages parents to  administer all medications at home. However, we recognize that students may need medications during the school day. Please review the school policy on medication administration.

Administering Medication Policy

If you student will need daily medication and/ or emergency medications during the school day, please submit the appropriate parent permission form to the school nurse along with the students medication.

Inhalers and Emergency Medications 

• for Student Possession of Inhalers or Emergency Medications.

• for medication administration, to be completed whether medication is prescription or non-prescription.
