Fall sports start Monday, August 12, 2013. Sport physicals are required for all students grades 7 - 12 only. All students who will be participating Club Sports grades 7 - 12 are also required to have a physical. The physical packet must be completed and return it to the Athletics Office in the High School.
Only one sport physical is required per school year and is due at the start of the sport season in which he/she will be participating. Please do not turn in or print a physical packet in the fall for a winter or spring sport since the forms may change. Sports physicals for the 2013-2014 school year must be administered and dated June 1, 2013, or later and will be valid until May 31, 2014.
Sports physicals will be provided on Saturday, June 8 at AGH Human Motion Wexford from 8 a.m. - 11 a.m. The cost is $10 and must be paid at the time of the physical, cash or check payable to HTSD. Please call 1-877-284-2000 to schedule your appointment. Other dates/times will be available if you are unable to make it to this physical. Fall sports physical packet
Fall sports include football (grades 7 - 12), cheerleading (grades 7 - 12), coed JH and varsity cross country (grades 7 - 12), JV/varisty golf (grades 9 - 12), JV/varsity boys and girls soccer (grades 9 - 12), JV/varsity girls tennis (grades 9 - 12), JV/varisty girls volleyball (grades 9 - 12) and 7/8 grade girls basketball. Club sports include coed varsity crew (grades 9 - 12), ice hockey (grades 7 - 12), inline hockey (grades 7 - 12), slow-pitch softball (grades 7 - 12) and ultimate (grades 9 -12).
All forms must be sent directly to the Athletics Office, not the coach or the Middle School office, prior to participation in the sport. Physicals turned in at the last minute may result in the student not being permitted to practice or to participate in tryouts. Such delays may result in a loss of tryout time or perhaps losing the opportunity to tryout completely. Once the physical is turned into the Athletics Office and reviewed for satisfactory completion, the student will be granted permission to participate in athletics. When a student-athlete has a desire to participate in a sport that has roster reductions or cuts, it is imperative that he/she has all of the paperwork submitted in a timely fashion.
For students who will be going to their own physician for a physical, the exam must be administered and dated on or after June 1, 2013, and will be good until May 31 of the following year. If the date on the form is incorrect, the physical will be returned and the athlete will be ineligible to participate until the physical is dated properly. We cannot accept any physical paperwork that is not provided on the PIAA or Hampton Athletic Website. Forms will be provided in the Middle School, High School and Athletics Office. Please do not turn in sport physical forms until the start of the season in which he/she will be participating.
